If you'd like to host a fundraiser, partner with Pat & Oscar's for a community event, or have a fundraising flyer created please fill out the request form below.

In requesting a fundraiser, please wait for approval from the restaurant before requesting your flyer. Once your fundraiser has been approved, you will need to fill out the form again and select the flyer request from the drop down menu. You may also download a W-9 form at the bottom of this page.
